Can a RIC application propose working with only a subset of CTSA hubs?
No. RICs will be expected to eventually work with all the CTSA hubs. Applications that propose limiting RICs to a subset of CTSA hubs will not be considered responsive to this FOA.

Yes. Phenotypic Drug Discovery Resource |
Through a unique collaboration with Eli Lilly and Company’s Open Innovation Drug Discovery (OIDD) program, NCATS researchers accessed Lilly’s panel of human disease-relevant assays, which are cell-based testing systems that help scientists explore the effects of small molecules on disease-related molecular processes. The approach, called phenotypic drug discovery, involves searching for agents that change cell function and is a common method for lead discovery in drug development.
Background
The collaborative team screened part of the NCATS Pharmaceutical Collection (NPC), which includes 3,800 approved and investigational medicines, against the OIDD assay panel, which was offered through Lilly’s OIDD platform. The OIDD testing panel consisted of assays relevant to Lilly’s areas of focus, such as cardiovascular diseases, diabetes, cancer and endocrine disorders, and was designed to help researchers uncover new starting points for early drug discovery.
Testing approved drugs in the OIDD assay panel broadens scientific knowledge of the biology of these compounds. Results from this effort may enable researchers to better predict treatment outcomes, design new drug combinations, and potentially reveal new uses for existing drugs, a time- and cost-saving approach called repurposing.

The OIDD Semantic Knowledge Network encodes the experimental relationship between NPC compounds and OIDD assays in a standardized format that can be exported into other databases and information sources. Similarly, it also encodes relationships between compounds and assays and among other experimental data, including ChEMBL dataset mapping and classification of assays using the bioassay ontology (BAO).
Users can download Semantic Knowledge Network data represented as resource description format (RDF) and save in the Turtle (.ttl) format. The beta release of these files, which includes everything in the first table below, provides the default graph of a knowledge network and one Web ontology language (OWL) file comprising one BAO module. In addition to the OIDD phenotypic assay data, protein targets and bioactivity associations from the ChEMBL database are included to facilitate queries that integrate phenotypic and biochemical data.

Stopping Metastasis in Its Tracks: New 3-D Cell Model Enables Closer Look at Cancer Progression |
For hundreds of years, scientists and physicians have struggled to fight cancer. In the past half-century, the technological advances of chemotherapy and radiation therapy have enabled doctors to shrink patients’ tumors, but all too often, the tumors return and spread to other organs by way of a deadly process called metastasis. Until scientists can better understand and model this disease process, a cure for cancer seems unlikely.
This challenge inspired Ernst Lengyel, M.D., Ph.D., department chair and Arthur L. and Lee G. Herbst Professor of Obstetrics and Gynecology at the University of Chicago; and Hilary Kenny, Ph.D., an assistant professor in the department, to contact NCATS. Lengyel and Kenny’s team had developed a new way to study ovarian cancer metastasis and potential treatments using a 3-D model.
Historically, scientists have developed disease models by growing cells in a single layer in a laboratory. This monolayer approach is easy to construct but does not represent the complexities of the human body. As a result, many compounds that seem effective in a monolayer culture on plastic do not pass animal and human studies for safety and effectiveness. To improve the odds of success, Lengyel and Kenny’s team constructed a 3-D model of ovarian cancer metastasis using cells from patients. The team needed to optimize the model for high-throughput screening, which would enable the researchers to test many different compounds at varied concentrations at a much higher rate than could be done manually.
For this next step, Lengyel approached NCATS’ Ajit Jadhav via NIH’s Molecular Libraries Program and established a collaboration that also included NCATS scientists Marc Ferrer, Ph.D.; Madhu Lal-Nag, Ph.D.; Min Shen, Ph.D.; and Matt Boxer, Ph.D. The team worked together to create, adapt and validate an innovative 3-D cell model used to identify small molecules that can prevent cancer cells from spreading to new sites in the body. The new model includes connective tissue cells and the extracellular matrix (a collection of supportive molecules outside cells), creating a more lifelike simulation of the human body environment. It represents an exciting advance in research technology, and the high-throughput screening results, published in the Feb. 5, 2015, issue of Nature Communications, illustrate the power of this scientific collaboration.
Getting to the Root of the Problem
Metastasis is a three-step process. First, cells from the primary cancer site travel to a new area of the body. Next, the cells latch on to healthy cells. Finally, the cancer cells invade those healthy tissues. This process of cancer cell migration, adhesion and invasion — not the original tumor — is the root cause of many patient deaths. A drug that prevents one or more of these stages of metastasis could effectively stop this deadly process in its tracks.
To advance toward this ultimate goal, the NCATS-University of Chicago team first adapted and miniaturized a 3-D ovarian cancer cell culture model so they could use it in the high-throughput robotic screening system at NCATS. Just a few of the 2,420 compounds screened in the assay (test), including one called beta-escin, demonstrated an ability to prevent all three metastatic stages both in vitro (in an artificial environment) and in vivo (in whole, living organisms). Beta-escin is a naturally occurring substance derived from the seed of the horse chestnut tree and is commonly used to improve chronic venous insufficiency, a condition in which veins cannot pump enough blood back to the heart.
“Beta-escin was a very surprising finding,” Lengyel said. “You can buy it online. It is currently used to treat vein disease, and its anti-metastatic effect was unexpected.” Although beta-escin has promise, Lengyel explained that it has not yet been validated in human studies and should not be considered a cure. “It’s a promising lead to pursue,” he emphasized.
Innovation in 3-D Modeling
In addition to the encouraging findings from the compound screening, this collaborative project yielded another important advance: the cell culture model itself. The new 3-D model has many advantages over its monolayer 2-D predecessor, and the compounds identified using the new model are more likely to succeed in in vivo trials compared with compounds selected from 2-D screens. Unlike monolayer models, the 3-D approach can reproduce cellular and genetic tumor variations, complex tissue architecture, and interactions within the tumor microenvironment.
“The 3-D model we developed is a more accurate representation of cancer cells in the human body compared to 2-D models,” Lal-Nag explained. “This approach improves our ability to find drug candidates that are more likely to be successful.”

By replacing generic cell lines with actual cells from ovarian cancer patients, the researchers introduced an element of precision medicine into the model. This specialized technique is particularly important for cancer treatments, for which the effectiveness of drugs varies widely from patient to patient. A personalized approach could reduce guesswork and unnecessary treatments.
“If we can take cells from patients and develop assays with those cells, the drugs we develop should work for that patient,” said Ferrer.
Applications Beyond Cancer
Although the team focused on ovarian cancer to demonstrate the power of the new 3-D model, this technology could advance translational research for diseases other than cancer. “It’s a general technology that translates to many different kinds of therapeutic approaches,” Jadhav said.
The potential for the new model also extends beyond therapeutic development: Its rich environment may offer basic scientists a new way of studying the underlying mechanisms of cancer and its progression. “The idea of migration, adhesion and invasion is fundamental to biology,” Jadhav said. “This could become a research tool for studying metastasis at the basic science level.”
The new model represents a different way of thinking about translational research. “Sometimes people think these type of assays are not possible,” Ferrer said. “I’m hoping this paper will change people’s minds — that the community will see this work and begin thinking beyond what’s usually done. We like it when people come to us with blue-sky ideas.”
Broadening the Scope
In the next phase of the project, the researchers will use the 3-D model to screen 400,000 additional compounds from another library. Validated compounds from this screen will be modified using medicinal chemistry to maximize strength and minimize toxicity before testing in animals.
“We hope there will be categories of compounds that act by different mechanisms, giving us an opportunity to explore these different classes of compounds,” Jadhav said.
This project exemplifies the power of collaboration for the benefit of scientists and patients alike. “That’s a common theme with NCATS projects,” Jadhav explained. “The passion that other investigators have for pursuing novel ideas is matched with the passion we have toward the same shared goal.”
“This advance would not have been possible without NCATS,” Lengyel added. “You would think that working with the government would be difficult, but this was seamless and a lot of fun. We are honored to work with such great scientists.”

Osteoarthritis is a disease of the joints in which the cartilage that normally covers the ends of the bones, reducing friction between them, wears thin. The result is pain and loss of motion or flexibility, which can become debilitating. Osteoarthritis is the most common form of arthritis, affecting more than 25 million Americans. Women are at greater risk for the disease than men. No cure is known for osteoarthritis, but treatments exist for the pain. These scientists are developing a gene therapy for osteoarthritis, which will be injected into the affected joint. The resulting gene product works by blocking an inflammatory agent that contributes to the deterioration of the cartilage. This can allow the cartilage to heal, restoring function in the joint.
Scientific Synopsis
The services requested in this application form part of the development of a Phase I clinical trial in the gene therapy of osteoarthritis (OA) and are needed to support an Investigational New Drug (IND) application. The proposed clinical trial is based upon a considerable body of prior research by the PI and other key personnel in which they have developed technologies permitting gene transfer to joints in a manner that gives persistent intraarticular transgene expression. They have demonstrated an anti-arthritic effect in animal models when interleukin-1 receptor antagonist (IL-1Ra) is used as the transgene. Based on this, they have developed a clinical protocol in which a self-complementing, recombinant adeno-associated virus (AAV) vector, serotype 2.5, carrying human IL-1Ra cDNA (sc-rAAV2.5IL-1Ra) will be injected into the knee joints of human subjects with mid-stage OA.
A detailed description of this protocol was submitted to the Food and Drug Administration (FDA) as part of pre-Investigational New Drug (IND) discussions. After several conference calls and exchange of information, the FDA gave a clear statement of what further preclinical studies are required before going forward with an IND application. This application requests resources to carry out the studies required by the FDA.

New Funding Opportunities for Trial and Recruitment Innovation Centers |
On May 15 and June 5, 2015, NCATS released two new Clinical and Translational Science Awards (CTSA) program funding opportunity announcements (FOAs), one for Recruitment Innovation Centers (RICs) and the other for Trial Innovation Centers (TICs). Both are aimed at overcoming key roadblocks to multisite clinical trials. CTSA hubs form a national network of medical research institutions that work together to improve the translational research process.Conducting multisite clinical research in the United States often is associated with implementation delays and high costs. This is due in part to long-standing challenges, such as historically low trial participant recruitment; complexity of obtaining approval of a single protocol by multiple institutional review boards; and few standards for executing research contracts, qualifying clinical sites and initiating study start-up procedures.“Through these new FOAs, NCATS is addressing systemic problems in translation and contributing to a collaborative national network for translational medicine,” said NCATS Director Christopher P. Austin, M.D. “These improvements are aimed at dramatically accelerating the clinical translational process and getting more treatments to more patients more quickly.”CTSA hubs collaborate to support high-quality clinical and translational research locally, regionally and nationally, fostering innovation in training, patient involvement and new methodologies. With these seven-year awards, NCATS anticipates spending $12 million in fiscal year (FY) 2016 to fund three TICs in response to the FOA, depending on the availability of funds. The National Institute on Aging will co-fund the TICs.The purpose of the RICs is to increase the likelihood of success for multisite clinical trials in two ways: (1) by developing informatics-driven approaches to assessing the site-specific availability of potential participants during trial planning and (2) by developing innovative approaches to participant recruitment during trial implementation. For these five-year awards, NCATS plans to fund up to two RICs in FY 2016 with up to $3 million each in total costs. News Brief: Allergy Drug Could Treat Hepatitis C |
A new drug can take about 14 years and $1 billion to develop. As one way to help speed the development of new disease therapies, rather than starting from scratch, NCATS researchers test existing medicines for their potential to treat other diseases.
Recently, NCATS scientists, in collaboration with researchers from the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), discovered that an over-the-counter drug used to treat allergy symptoms also limited hepatitis C virus (HCV) infection in human liver cells. The findings suggest that the drug, chlorcyclizine HCl (CCZ), potentially could be used to treat this chronic infection in people. Results were published April 8, 2015, in Science Translational Medicine.
An estimated 185 million people worldwide suffer from HCV infection. It causes liver inflammation and often leads to serious complications such as cirrhosis, which can be fatal. Early diagnosis and treatment of HCV infection can prevent liver damage.
The NCATS researchers (see sidebar) collaborated with NIDDK’s Jake Liang, M.D., in using the Center’s high-throughput screening capabilities to look for compounds that blocked viral activity in HCV-infected human liver cells. The team tested the NCATS Pharmaceutical Collection, a library containing approved and investigational drugs, and identified CCZ, an antihistamine, as a promising candidate that appeared to limit the early stages of HCV infection. The effect was enhanced when the researchers combined CCZ with existing HCV drugs, suggesting CCZ could be an effective add-on to current treatments.
Next, the team gave CCZ to HCV-infected mice and found that the compound significantly reduced viral infection. The treated mice showed no signs of drug resistance, a common problem among existing HCV therapies. The study results suggest CCZ, with its well-established safety profile as an allergy drug, could be a more broadly accessible treatment for HCV infection.
Scientists still have much to learn from approved drugs and their untapped potential to help researchers understand basic biology and develop treatments for unmet medical needs. In addition to HCV, other NCATS repurposing screens have identified potential drug candidates for a variety of illnesses, from blocking Ebola virus infection to reversing multiple sclerosis.
The study in mice provided data that validated chlorcyclizine’s activity. Based on the results, the team worked through the Food and Drug Administration to quickly move this discovery from the lab to testing of CCZ in patients with HCV infection at the NIH Clinical Center.
