At NCATS, these robots test thousands of potential drugs and drug combinations, environmental chemicals, and other experimental compounds to gain insights about a wide range of human diseases.Back in 2010, staff at what is now the NCATS high-throughput screening facility were supporting the NIH Molecular Libraries Program, an NIH Common Fund initiative to identify small molecules that potentially could be used as biological probes to explore the basis of health and disease and further developed as drugs. At the end of each set of several thousand experiments, a staff member would remove a stack of plastic plates, each about the size of an index card and a half-inch thick.“We’d throw them in here,” said Sam Michael, NCATS director of automation and compound management, pointing at a “carboy,” a waist-high blue plastic barrel used to collect chemical waste. “We were so busy, there were about 15 of these — I literally didn’t have enough room to work.” Michael also is the acting director of the NCATS Office of Information Technology.The NCATS high-throughput screening facility is part of a laboratory where technicians work among tables and shelves stocked to the brim with equipment, plates and cables, leaving little space for more than a dozen barrels of waste. Michael knew there had to be a better way, and he found one: washing and reusing the plates.Although that solution may sound trivial, it required persuading skeptical biologists that the plates could be cleaned thoroughly and rethinking the entire process of high-throughput screening. Today, experiments that once took thousands of plates can be run with just 40. This innovative idea has saved hundreds of thousands of scientific research dollars and dramatically reduced environmental plastic waste. And now, through a Small Business Innovation Research (SBIR) program grant from NCATS, a private-sector company is building on Michael’s work to develop and commercialize an automated plate cleaner.“That Will Never Work”Biologists are accustomed to working only with new materials. The high-throughput screening process involves sensitive experiments with tiny droplets of liquid; results could be completely thrown off by the merest speck of residue. Michael explained, “When I told biologists, ‘We are going to try to reuse your plate within an experiment,’ the first reaction was, ‘Get out of here; no way!’” But he inevitably convinced some of the researchers to try it.If you held a three-by-five-inch plate in your hand, you would see a fine grid of tiny wells, each about as big around as a pencil lead. A plate the size of an index card holds 1,536 wells. Together, all of the wells in one plate hold about a tablespoon of liquid. That may seem small, but the wells are big enough for chemical reactions. Each one is like a tiny test tube, holding a tiny experiment.For high-throughput screening, a plate starts at a dispenser where small tubes spray the same substance into each well. Sometimes the plate wells hold living cells; during a visit on a late winter Wednesday, enzymes were on deck. Robotic arms transfer plates from one machine to another in assembly line style, with quick, efficient movements and the whirring sounds of precise motors.The next stop for the plate is a machine with a rack of 1,536 tiny metal pins, one for each well. The pins act like a rubber stamp; the “ink” is a plate of compounds from a library of small molecules. The pins dip into the compound plate and then into the wells on the experimental plate, delivering one compound to each well. Each well gets a specific type of molecule that will interact with the enzyme, then the plates sit for a few hours. Finally, another machine takes a picture of each plate so scientists can figure out how each compound affected the reaction in each well.A Clean StartFollowing the traditional process, the plates become trash after imaging; they go to a landfill or incinerator, depending on the kind of waste they contain. But in the process Michael created, the next stop is cleaning. A dispenser alternates spraying deionized water and ethanol into the wells. After each spray, a row of aspirators, like the world’s tiniest vacuum cleaners, passes over the plate to suck the liquid out.The cleaned plates are now ready to go back into the screening process. A plate is only reused within the same assay, or set of experiments. In practice, half of the plates for an assay are being cleaned while the rest are in testing.Before starting each new assay, the NCATS team screens a few test plates to ensure cleaned plates give the same results as new plates. Sometimes the particular chemicals involved in the experiments just can’t be washed out, so the plates can only be used once, the old-fashioned way.Once the assay is complete, the plates are always discarded, but the savings from reuse during screenings are huge. At about $10 per plate, an experiment that previously involved 2,000 plates at a cost of about $20,000 in plastic now costs $400 for those materials — 2 percent of the original expense.So far, this method has saved NCATS almost half a million dollars and kept nearly 50,000 plastic plates from filling up chemical carboys. If that many plates were laid out end to end, they would be taller than Mount McKinley, the highest mountain in North America. The solution also has saved staff time from loading and unloading plates: four solid work weeks, Michael estimated.Disseminating the TechnologyThe creation of the plate-cleaning technology aligns perfectly with the NCATS “3 Ds” approach to translational science. Michael developed an innovative solution to address a translational problem and then demonstrated that the approach worked and could save time, money and environmental waste. The next step was disseminating the technology to benefit the larger scientific community.To reach that audience, Michael thought a small business might be able to scale up and expand the plate-cleaning technology. The Center’s SBIR program helps U.S. small businesses develop and commercialize new translational technologies. Michael worked with Lili Portilla, M.P.A., director of the NCATS Office of Strategic Alliances and SBIR program manager, to issue a contract solicitation for plate cleaning.IonField Systems in Moorestown, New Jersey, responded to the solicitation and secured a Phase 1 SBIR contract from NCATS, then received a follow-on Phase 2 award. IonField already had developed a method called “plasma washing” to clean plastic tips used to move small quantities of liquid in biochemical research. To adapt this method to cleaning plates, the company put a final touch on the process that Michael had envisioned, adding a plasma wash after the rinses with ethanol and water. High-voltage electricity eliminates anything still attached to the plate and even scours off the top layer of the plastic itself.The ultimate plan is to sell the cleaner to academic and industry screening facilities and offer a service for cleaning plates. The development of the cleaning process and service has turned what was once disposable waste into a reusable resource disseminated to other laboratories and pharmaceutical companies nationwide.“This is exactly what the NCATS SBIR contract program is designed to accomplish,” Portilla said, “NCATS identifies a translational technology challenge that small businesses can address, commercialize and disseminate into the marketplace.”“The SBIR program is a great mechanism, and I’m glad somebody can take this NIH technology and make it available to other scientists to speed their high-throughput screening activities,” Michael said.The success of the plate-washing method demonstrates NCATS’ commitment to transforming the way that translational science is conducted. 2980 New CTSA Program Funding Opportunities: Collaborative Innovation Awards On April 2, 2015, NCATS released two new funding opportunity announcements (FOAs) for Collaborative Innovation Awards, designed to stimulate team-based research across the Clinical and Translational Science Awards (CTSA) consortium. NCATS’ CTSA program supports a national network of medical research institutions — called hubs — that work together to improve the translational research process to get more treatments to more patients more quickly.Translating biomedical discoveries into the clinic is essential to improving human health. However, the current translational process has many barriers, resulting in long delays before a new treatment is available to people who need them.PAR-15-172 and PAR-15-173 solicit proposals for innovative collaborative investigations among three or more CTSA hubs to improve research methods at any step in the translational process. Through these awards, NCATS will foster research collaboration by encouraging teams from multiple hubs to work together to develop, demonstrate and disseminate multisite experimental approaches that overcome translational science roadblocks.“By combining the innovative efforts of three or more CTSA hubs in flexible networks, we aim to enhance the effectiveness of the CTSA consortium to address systemic translational science questions,” said Petra Kaufmann, M.D., M.Sc., NCATS director of clinical innovation.PAR-15-173 solicits pre-applications from interested investigators. 2355 NCATS Support Leads to Clinical Trial to Test Repurposed Cancer Treatment as Alzheimer’s Therapy As Baby Boomers get older, the number of people with age-related conditions such as cancer and Alzheimer's disease continues to grow. Alzheimer's disease is the most common form of dementia, a group of disorders that cause progressive loss of memory and other mental processes. About 5 million Americans have Alzheimer's disease, and current drug therapies can only ease symptoms of the disease without stopping its progression. New treatments — so-called disease-modifying therapies — are needed to halt Alzheimer's by targeting its underlying mechanisms. Blocking that path to therapeutic success is the costly, complex process of drug development. The average length of time from discovery of a therapeutic target to approval of a new drug is about 14 years. The failure rate during this process exceeds 95 percent. NCATS is addressing these translational bottlenecks through programs such as the Discovering New Therapeutic Uses for Existing Molecules (New Therapeutic Uses) program. Launched in 2012, this initiative matches academic researchers with pharmaceutical industry assets that have undergone significant research and development to accelerate the process of finding new therapies. Now, NCATS is celebrating one of the first promising results from the New Therapeutic Uses program: Center-supported scientists at Yale University School of Medicine have found that an experimental compound originally developed as a cancer therapy potentially could be used to treat Alzheimer's disease. The compound successfully reversed brain problems in mouse models of the condition, and now the researchers are testing it in humans. The results of the animal study were published for early view on March 21, 2015, in the Annals of Neurology. Read the NIH news release. A New Therapeutic Target Back in 2012, Yale neurobiology researcher, neurologist and senior author of the animal study Stephen Strittmatter, M.D., Ph.D., and his colleagues found an important, missing piece of the Alzheimer's puzzle. They wanted to understand more completely the molecular events that occur in the brain to produce symptoms. In Alzheimer's disease, abnormal clumps of amyloid beta protein build up in the brain. These protein clusters damage brain cells (neurons), eventually killing them. However, how amyloid beta harms cells has been unclear. The Yale team discovered that aggregated amyloid beta activates a series of signals within neurons that leads to abnormal functioning and loss of synapses, which are the spaces between neurons that enable the cells to "talk" to each other and form memories. Central to this process is the activation of a protein called Fyn kinase through another molecule, cellular prion protein. These results suggested that a compound that blocks Fyn activity might represent a potential disease-modifying therapy for Alzheimer's. The Power of Crowdsourcing Around the time Strittmatter's Fyn finding emerged, NCATS launched the New Therapeutic Uses program and released a list of pharmaceutical industry assets, inviting scientists to pitch new ideas for diseases that might be treated with those assets. That 2012 list included a Fyn kinase inhibitor, saracatinib (AZD0530 [PDF - 65KB]), developed by biopharmaceutical company AstraZeneca. Strittmatter, along with co-principal investigators Haakon Nygaard, M.D., Ph.D., and Christopher van Dyck, M.D., submitted a proposal to test the hypothesis that saracatinib could improve Alzheimer's-related brain abnormalities. The team received one of the first New Therapeutic Uses awards in June 2013. "AstraZeneca developed saracatinib to treat cancer outside the brain, so nobody had thought to link it to Alzheimer's disease," Strittmatter said. "This connection — between our knowledge of Fyn kinase in the brain and AstraZeneca's information on this compound — would never have happened without the New Therapeutic Uses program." (Credit: Strittmatter Laboratory, Yale University Photo/Adam Kaufman) A Promising Effect in Mice AstraZeneca scientists teamed with the Yale group, providing saracatinib for the mouse study and sharing knowledge and data gathered from previous studies. "No one individual or group has complete knowledge of disease pathways and treatment targets," said Craig Wegner, Ph.D., Head, Boston Emerging Innovations Unit, Scientific Partnering & Alliances within AstraZeneca's Innovative Medicines and Early Development Biotech Unit. "This program successfully unites scientists from government, academia and industry and is a great example of how we are working together to push the boundaries of science." The Yale team gave the experimental drug to mice with Alzheimer's-like symptoms, such as memory problems and age-related buildup of abnormal amyloid beta clusters, modeling the development of the disease in humans. After four weeks, the Alzheimer's mice showed complete reversal of spatial learning and memory loss. When the scientists examined the brains of the mice, they found that the characteristic synapse loss had been fully restored, providing a biological explanation for the memory improvement. The treatment also reduced several other Alzheimer's-related biochemical changes in the mice and did not appear to be toxic. Although many experimental treatments have aimed to reduce abnormal amyloid beta buildup in the brain, this one is unique in that it targets the toxic effects of the protein clusters within a cell, appearing to protect it from damage. The Yale research team also has completed a successful Phase 1b safety study of saracatinib in humans with Alzheimer's disease, showing that the compound reaches the brains of patients at levels similar to those beneficial in mice. The study results have been accepted for publication later in 2015. The data are encouraging, but the researchers cautioned that more human studies are needed to determine if saracatinib is an effective treatment for Alzheimer's. An Accelerated Path to the Clinic Saracatinib's prior development and the Yale team's successful completion of animal and human studies enabled the compound to advance rapidly into a larger, multisite Phase 2a trial in Alzheimer's patients now ongoing. "The speed and efficiency with which this research has advanced has set new standards of excellence, enabling us to jointly push the boundaries of medical science," Wegner said. Using the pre-negotiated NCATS template agreements, which were designed to streamline the legal and administrative process for research collaboration by multiple organizations, "really facilitated our collaboration with Yale, so scientists could be scientists," he added. "Through this project, NCATS and AstraZeneca have provided us with an incredible shortcut in the drug development process and have accelerated the path to finding a more effective Alzheimer's disease treatment," Strittmatter said. In the Phase 2a trial, 152 participants will receive saracatinib or placebo for one year. Researchers will assess safety, tolerability and effectiveness of the experimental drug, and they will use brain imaging to visualize the effect of saracatinib on synapse function — the same feature improved by the drug in mice. Study investigators currently are enrolling older adults with Alzheimer's disease to participate in the trial and expect to have results in about two years. The Phase 2a study will take place at multiple clinical sites as part of the Alzheimer's Disease Cooperative Study, an initiative for multisite studies sponsored by the National Institute on Aging (NIA) to facilitate the development and testing of new therapeutics for the condition. In addition to funding from NCATS, the NIH Common Fund, NIA, BrightFocus Foundation, Alzheimer's Association and Falk Medical Research Trust provided support for the animal study. "The Yale team's awareness of this new Alzheimer's drug target, combined with AstraZeneca's drug development resources, allowed the rapid advancement of saracatinib to clinical testing, demonstrating the power of NCATS' New Therapeutics Uses crowdsourcing approach," said NCATS Director Christopher P.
